Hospital Discharge

Leaving the hospital is an important step in the recovery process, but getting home safely can sometimes be a challenge. Many patients are discharged after surgery, illness, rehabilitation, or medical treatment and may not be physically able to drive themselves. Others may require assistance entering and exiting a vehicle, help with mobility devices, or transportation to a rehabilitation center, assisted living community, or family member's home.
